DermTV – Perleche: Cracked Corners of Your Mouth [ Epi #156]

Hello, I’m Dr. Neal Schultz [pause] and welcome to DermTV. Itching and burning cracks right at the corners
of the mouth is kind of common, it’s uncomfortable, and it’s called
perléche. Because of the cracks, most people think it’s dry skin,
but it’s not dry skin. It’s actually caused by a yeast infection, and
it’s the same yeast that most women carry in their body and the same yeast
that causes them uncomfortable itching yeast infections. Because this is
caused by a yeast, it is aggravated by moisture, because yeast thrive
in moist areas and closed areas, and this is a perfect set-up because
at the corners of the mouth, the way the lips come together, they cause
a closed area and, if there’s any saliva there, that accounts for additional
moisture. When people get this, invariably they do two things that make
it worse. The first is, because they consider it to be dry because
they see a crack, they put a moisturizer on and, again, moisture makes
the yeast grow even more. Secondly, because it’s uncomfortable, and
it burns, and it really feels peculiar, a lot of people take the tip of
their tongue and they go like this. And they do it to test it, to feel it,
to see, is the problem still there and, interestingly, people do it very
often, a couple of times a minute, it’s just a natural reflex. So how do we cure this problem? The first
thing is obviously stop licking the corners of your mouth. The second is,
get an over-the-counter anti- yeast medicine, such as Lotrimin which has
clotrimazole as the active ingredient which is good to fight yeast. Put
a little bit on the corners of your mouth four to six times a day, just a
very small amount, and then put a little Vaseline or Aquaphor on top so, if
you do accidentally touch it with the tip of your tongue, the Aquaphor
or Vaseline will keep the moisture away from the problem. Give it four
to seven days to cure the problem. If it hasn’t done that by that
period of time, then you should see a dermatologist who will give you a prescription
for an anti-yeast in an ointment vehicle which again will help protect
against additional moisture which aggravates the problem.

  1. Have this issue, went to see my dermatologist three times with different meds, last one is just a bleach (hydroquinone) but the discoloration is still getting larger. I hope this thing you talked about in this video will work on me. I will see my dermatologist again and talk to him about your video. I am very hopeful your suggestion will work on my case.     Do u agree B-12 deficiency causes this, too? I have it but less visible than your photo and often my whole lips flake & peel. 2 doctors said it's dryness or vitamin deficiency. I was skeptical, since I live in the NYC area and get this all year. Plus I take oral vitamins. However, when I agreed to vitamin injections, this issue resolved.

  3. try an antifungal cream (for athlete's foot), if it doesnt work, try monistat or canestant. that what my dermathologist told me.

  4. Hi Dr. Schultz, I've been having dry peeling lips for 3-4 weeks now and it hasn't healed yet, no matter how much lip balm I put on. I also feel burning sensation at the corner of my mouth but I can't really see any cracks. Please help me, is it perleche? how do I treat my dry lips as well?

  5. I moved to a different climate 21 years ago, than I spent my first 26 years in. For some reason, I started getting this problem; it had never happened before. My doctor gave me an antifungal called Loprox 1% Ciclopirox olamine. He told me to apply it twice a day, but I've been using it more often. It's helped a little. My question is because it's a yeast infection, can I use diflucan (don't need a prescription anymore)? Will that work? I'd much rather take one pill & get it overwith. Thanks!

Omg i have like literly 25 cracks on the both sides of my mouth and it gets very dry and when i open my mouth it hurts so much and sometimes it bleeds but only a little and it burns and lip balm doesnt help me and like when i open my mmouth the cracks show and it looks disgusting is there a way to fix it

  7. hi, i am really red at the corner s of my mouth, and i put on alot of moisturiser. and if i stop, i get cracks and so on instead. should i try that medicine you mentioned in the video? Ive read alot and people claim it could be caused because of Ecsema, lack of iron and b12 and other things. is this perleche?

Thank you!!  I am a diabetic and am actually fighting my primary physician to put me on a fast acting insulin…on a long lasting and it isn't covering…anyway I have noticed this issue since my blood sugars have been well in the 400's for the past 3 months!!  Time to switch doctors….I know yeast infections increase in a human body if blood sugars stay high for too long!

  9. I was using the Lotrimin (generic) but not 5-6 times a day…so that's going to change! What I discovered for me is that any toothpaste with mint in it irritated my lips and corners and same with eating something salty (like potato chips) and then licking my lips…ow!  So I found this fab organic minimal ingredient toothpaste for kids at my "health food store" and I like Not having a mouthful of toothpaste foam and no burning. It may be caused by a yeast infection, but some foods/products can irritate further and cause more swelling and redness…I also have it on my lower lip.

  10. Can anyone help me please? I had this so long ago and it was horrible. I couldn't open my mouth to eat without intense pain. As soon as it was healed, I got dark spots under the corners of where my lips meet. 3 years later, I still have these dark spots. So hard to conceal with makeup and needs to be reapplied often. I've tried so many fading creams and even hydrogen peroxide. Nothing seems to lighten them:/ what do I do?? Very insecure about it.

  11. Thank you, I get pimples sometimes only on the corners of my mouth. Is there a reason they only show up there? I have a hard time concealing them with concealers because they often rub off while I move my mouth while I am talking. :*( I used acne remedies on them but unlike the rest of my face the remedies dry out that area and it starts to crack!! Then it looks worse because that area is constantly moving with my facial expressions. Then it starts to look like I have that yeast or herpes, but its just acne 🙁

  12. Uhhh, Angular chelitis is not always caused by a yeast infection…what if it's bacterial or caused by contact dermatitis?

  13. This doctor is as stupid as most of them, who learn only to name the disease, and connect it with medication….. Bulshit 😉 I have noticed when I eat bread (I usually eliminate it altogether) it heals itself. Some breads do this – German rye in Trader Joe, olive bread in fresh and easy, and even french baguette  from Ralphs stores.. I assume I am lacking some vitamins B complex and I would like to know better remedy, aside from cleansing my body and allow for self- regulation all together, accordingly to a school of Natural Hygiene. I am still searching, and usually will find  possible connection, nutritionally 😉

Honestly i needed to thank this dr for this video. i went to two  (pls one plastic surgeon) reputable for this conduction and ALL of them didn't know what it was! i swear….i just watched this by accident after seeing the picture. i just bought this medication and fingers crossed i will use it starting tonight..thanks for at least knowing what it is!

Or go and get yourself a no scent gel hand sanitiser put a tiny blob on each corner preferably before bed and by morning it's gone, I stumbled on this remarkable cheap fool proof solution by accident. And I can truly say it will sting terribly at first then it will go numb but its worth it.👍😆

  25. Why prescribe Lotrimin? Are you being paid to hawk that do nothing shit? Why not tell the truth; that Monostat with miconazole is the silver bullet needed to do the job. I'm a guy. The last thing I would think of is to use a women's vaginal cream (SHIT, can that make you GAY?), and Lotrimin has NEVER worked on jock itch or foot fungus for me. I live in Florida by the way. I treated this presentation in the corner of my mouth with monostat and after 2 days it is 50% to 75% GONE! LOTRIMIN SUCKS! It is like Tylenol as apposed to Goody Powders or 800mg Ibuprofin. NO COMPARISON.

  28. here's a few ideas for lessening angular cheilitis
    Honey: Honey works well in fighting the infection-causing microorganisms. Apply honey on the angular cheilitis and leave for 15 minutes. This may also be used alongside cucumber.
    Aloe Vera Gel: For treating and alleviating the pain of angular cheilitis, rubbing aloe vera on the cracked corners of the mouth also proves to be helpful. Refrigerate a leaf of aloe vera before to applying it on the affected area. Rub it for a few minutes and let it dry for at least 20 minutes before washing it . This treatment for angular cheilitis is 100% natural)
    Moisturizers and Lip Balms: Unscented and unflavoured lip balms, like petroleum jelly or virgin coconut oil heal the infection and are an excellent natural treatment for angular cheilitis. Ensure that there are no additives present in the balm or moisturizer. Do this as many times as you can in the daytime and right before sleeping.
    Water: “Hydrate yourself” is the best way for treating yourself with angular cheilitis. consume plenty of water, so that you do not get chapped and dry lips, which eventually develop cracks. Keep the body hydrated by consuming ten or more glasses of water daily to cure cracked lips.
    Cocoa Butter: To get rid of dry lips and eventually avoiding the problem of chapped lips, cocoa butter is a great moisturizer. Apply a small amount of natural cocoa butter, 2 or 3 times a day on the affected area, for treating angular cheilitis.

  30. No lie I've had this shit going on for the past YEAR!!! And it keeps splitting back and forth left and right corners. UGH!! It would heal then come right back, heal then come back over and over again.

  31. i bit my lip and ended up with a crack that won't heal ; probably infected of bacteria or yeast
    tried several alt med stuff just got smaller and less sore but did not heal 100% until i tried the baby diaper rash ihle's paste heal overnight. I get better results just trying several thing until the best result happens. The scientific approach is a thinking process can easily be misled

  34. I have cracked corners on my lips but I'm almost positive it's not a yeast infection because it does not ich or anything it just kind of hurts and is dry the chap stick seems to help but there not going away

  35. when i get it, there is always correlation to it and being stressed or a lot of exertion. Like working a bunch. It goes away by its self with rest and regeneration. It is a good tip to keep it dry and baby it, like dont split it more by opening your mouth too big and stuff.

  37. Whatever you do, do not put carmex or Chapstick on your lips. This will contaminate the other side of your lip as well as the tube of carmex/Chapstick causing a repeat cycle. Use the lotromin (there are several different formulas, be sure to use the one with Clotrimazole cream) and the vasoline combination as directed in this video, especially right before bed. If this does not work, use Neosporin plus on the sides of the lips only, as if it is not fungal, it could be bacterial. Be sure to take your vitamins as well. This is NOT a case of chapped lips. You can have the softest lips ever with this reoccurring problem.

  39. Ok I’m lost because I have cracks in the corners of my lips but it doesn’t itch or burn. My dentists and orthodontists haven’t said anything I also have cracks on my tongue but they don’t burn either. When I wake up and smile or like any movement with my mouth they crack and leave red marks after healing sometimes it’ll go away for a few days but just comes back this has been going on ever since my braces. I think and whenever I go to the ortho or dentist they stretch my lips and it cracks it’s not very painful just the red marks are annoying someone helpppp

  40. Yeast can grow anywhere in the blood, or topically, on your scalp, in your ears, on your feet. Yeast becomes an issue when your immune system is compromised and it cannot fight off the yeast so it overgrows. This can happen literally anywhere on or in your body. Not everyone one here could have yeast infections on their mouth but it can happen. It can stay for years if you go to bed late and have little as 5 hours sleep a day, too much stress etc. To cure this you should have 8-10 hours of sleep a night, cut out sugar out of your diet, try topically applying baking soda, apple cider vinegar (will sting and burn) and lower stress in your lifestyle. This problem as well as other yeast related problems in the body will disappear and your body will be back in balance
